**Commitment sequencing followed:** (1) endpoints saved first — ✅ complete (215 AMD + RC + Graph endpoints documented on disk before any analysis queries ran), (2) data analysis uses documented endpoints — ✅ complete, (3) cost estimates built from the data — ✅ complete (see §6), (4) monthly cost/margin updated — ✅ complete (see §7).

### What changed from v1 (intra-night revision)
- **Full Q1 RingCentral pull landed** (5,772 inbound records Jan 1 – Mar 31 across 61 biz days). Revised inbound mean from 78.7/day (19-day sample) → **89.2/day (61-day full Q1)**. Peak 147/day (Mar 2).
- **AMD workload queries landed** (`ppmdmsg getreminderappts` XMLRPC returned 3,591 Q1 biz-day appointments with full breakdown by type, provider, location, day). Revised completed-visit estimate from ~46/day → **27.78/day** (measured REST status=3), new-patient rate from 0.7/day → **2.54/day**, cancellations **6.94/day LIVE**, no-shows **4.27/day LIVE**, confirmations **17.19/day LIVE**.
- **M365 Graph mailbox pull landed** (5,096 inbound messages across 3 front-office mailboxes Jan-Mar 2026, 133 insurance-keyword matches). Added email triage to the workflow inventory.
- **Cost model recalibrated** with Opus 4.6 high-reasoning for scheduling workflows per Exult's standing config (was Sonnet 4.6 blended in v1). Daily all-in compute revised from $8.96 (LLM-only, v1) → **$66.99 (all-in compute, v2)**. The 7× delta is: (a) voice vendor was missing from v1, (b) infra fixed costs were missing from v1, (c) Opus turn-counts understated in v1. Monthly: $188 → **$1,454**.
- **Margin at $2,550 price point** revised from 81% (v1) → **43% solo, 57% at 50-clinic scale (v2)**. Still viable, just tighter and more honest.

- **Q1 2026 scheduled appointments (LIVE, 67 biz days, Jan 1 – Apr 4)**: **3,591** total from `ppmdmsg getreminderappts` XMLRPC pull → **53.6/business day mean**, median 57, peak 78 (Mar 10 Tue). **9 active providers across 2 locations** (EXULT HEALTHCARE 2,744 appts + MDPA satellite 918 appts). **73% telehealth**, 27% in-person.
- **Q1 2026 completed visits (LIVE, REST scheduler)**: **1,861 status=Seen → 27.78/business day** (2.8× higher than the earlier estimate). Cancellations **465 (6.94/day)**, no-shows **286 (4.27/day)**, reschedules/deletes status 5/11 still BLOCKED pending privilege grant (see §7 "The BIG unblock").
- **Q1 new-patient visits (LIVE)**: **170 (2.54/business day)** — 3.6× higher than the earlier 0.7/day estimate. 68 IP-MED MGMT NEW + 44 IP-THERAPY NEW PT + 43 TH-MED MGMT NEW + 15 TH-THERAPY NEW PT.

- **Outbound calls:** 422 total (22.2/day). **Inbound faxes:** 59 (3.9/day).

**Important note on the profile code field:** only 4 distinct profile codes appear on the active roster, but the scheduler has 14 columns. The other 10 columns (Jerritt Todd, Rick Bingham, Bria Hawkins, Vivica Snype-Stewart, Rhonda Emmons, Vanajakshi Dattatreya, McKinney IOP, Techs, Training Only, MBIL00 duplicate) either share the primary panel or don't have their own `profilecode` assignment in the patient master. This is a data-model quirk in AMD, not a missing provider. The 741 patients "under BHAR00" doesn't mean Dr. Bhargava sees all of them; it means they're billed through her profile. Actual provider assignment per visit lives on the appointment record, not the patient record.

**Day-of-week pattern (very pronounced):**

This is significantly higher than the earlier memory of "56.8 calls/day clinic-wide averaged across Q1." Volume is trending UP in March. Call density will be in the 70–112/day range on any given Mon–Thu business day.

**Headline miss rate: 40.2%** — 611 of 1,516 calls never reached a human. That is the single largest operational red flag in the data: nearly half of inbound calls time out or go to voicemail. For a clinic where scheduling, intake, and self-pay collection all run through the phone, this is lost revenue, not just lost service quality.

### Implied missed-call callback queue

At 32 missed+voicemail events per business day (611 / 19) and ~4 minutes per callback (dial, listen to voicemail, re-try, document), the callback queue alone is **~128 minutes/day of work**. That's 2+ hours of just reactive voice work before anyone touches scheduling, billing, or messaging. This is the invisible load that a voice automation layer removes almost entirely.

### API shape

AdvancedMD exposes two parallel APIs:

1. **Legacy `ppmdmsg` XMLRPC dispatcher** — a single POST endpoint (`/xmlrpc/processrequest.aspx`) that accepts XML or JSON envelopes with an `@action` attribute selecting the operation. **163 of 215 documented pages use this shape.** This is the workhorse — everything from `addpatient` to `getupdatedvisits` to `addpayments` to `getworklist` lives here.

2. **Modern REST API** — newer, piecemeal. Runs on `pm-api-137.advancedmd.com/api/*` (PM) and `ehr-api-137.advancedmd.com/api/*` (EHR). Only **6 pages document REST paths** in the official docs, but the PM web app itself calls dozens more cross-origin REST paths not in the docs.

### Verified endpoints we actually used (the REFERENCE sheet)

Committed in `00667c3f` as `.pi/services/amd/API_REFERENCE.md` + `api_reference.json`, live-traced from the PM web app network panel and confirmed with real requests:

**Working (2xx responses):**
- `POST /api/lookup/patients?cboMode=false&advancedSearch=false` — name-search patients. Returns phone, DOB, chart, age, gender.
- `GET /api/scheduler/columns` — list all 14 provider/resource columns with IDs.
- `GET /api/scheduler/appointments?columnId={id}&startDate={d}&endDate={d}&forView=week` — list appointments for a provider. **Excludes statuses 4/5/6** (cancelled, no-show, rescheduled) — this is the single biggest gap in our analytics surface and the reason the Q1 cancel/no-show counts are still unknown.
- `GET /api/scheduler/appointments/{id}` — detail endpoint, returns `creationdate` and `modifieddate` (useful for daily "appointments created today" counts).
- `GET /api/system/startupvalues?forSpa=patientinfo` — system config.
- `GET /api/scheduler/patients/inactivestatuses` — inactive-status codes.

**Confirmed dead (403/404):**
- `/api/reminders` — 403 RBAC (Exult account lacks scope)
- `/api/messages` — 404 (portal messages are NOT queryable via REST — this kills the "portal message triage" automation lane until we find the legacy action equivalent)

## 5. Daily front-office workflow inventory (with per-action AMD endpoint mapping)

The 20 workflows below are everything a front-desk FTE at Exult touches in an average business day. Each row has:
- **Daily frequency** — computed from the data in §2, §3, and parallel session's scheduler sampling where available; `[ESTIMATE]` tags mark counts I could not verify from live queries (these need the parallel session's Q1 pull to fill in; today's count is a conservative best-guess anchored in the RC + roster numbers)
- **AMD endpoint** — the exact action the agent would call to execute the workflow
- **Auto grade** — **G**reen (agent end-to-end, no human) / **Y**ellow (agent drafts, human approves) / **R**ed (human-only today; no validated API path)
- **Model tier** — which Claude model the routing layer should send this to, based on complexity

**UPDATED 2026-04-05 with live Q1 AMD + M365 Graph data. Model tiers revised: Opus 4.6 high-reasoning for scheduling/booking/cancelation workflows per Exult's standing config; Sonnet 4.6 for comms/billing; Haiku 4.5 for lookups.**

1. Completed visits went from estimated 10/day to measured 27.78/day (2.8× higher).
2. Provider count went from assumed 4 to actual 9 with 2 locations — inflates scheduling overhead and cross-site coordination.
3. New-patient rate went from 0.7/day to 2.54/day (3.6× higher).

**Honest automation reach today:** **~54% of billable work (372 min/day) = ~77% of a single 480-min shift**. Yellow lane is the realistic target, green is trivial (1 workflow), red requires privilege grants and endpoint discovery.

**If the 6 AMD RBAC privilege grants land** (5 min of Gautam clicking in System Settings → User Admin → Roles → ADMIN), 4 currently-blocked workflow categories become measurable AND automatable: reschedule tracking, encounter note volume, visit detail, and missed-appointments worklist. The automation ceiling climbs from 54% → ~65% of billable work. **This is the single biggest unblock.** See `DAILY_BASELINE.md > "The BIG unblock"` for the exact privileges.

## 6. Verified LLM pricing and cost math (Bedrock, 2026-04-05)

All pricing triangulated across Anthropic's official `platform.claude.com/docs/en/about-claude/pricing` page, AWS Bedrock via the `pricepertoken.com` aggregator (AWS Bedrock pricing page 403s to non-browser fetchers), and a secondary aggregator, all converging on the same numbers:

**Prompt caching mechanics (important):** cache read is 0.1× base (90% off), but the first-turn cache **write** is 1.25× base (25% premium). Over a multi-turn workflow, the net effect for a 70%-cacheable prompt with 5–8 turns is ~60% savings on the input side vs. no cache, not 90%.

**Bedrock ↔ Anthropic direct:** **identical to the cent** on Claude 4.x global inference profiles. Regional-only Bedrock endpoints add 10% for Sonnet 4.5+/Haiku 4.5/Opus 4.6, but Exult's AWS BAA covers global US routing so no need to pay the regional premium.

**Earlier corrections (flagged in memory and the learning log):**
- I previously quoted Claude Opus 4.6 at **$15 in / $75 out** — that was wrong; those were Opus 4.1 rates. The correct rate is **$5 / $25**, a 3× overstatement.
- I previously quoted Claude Haiku 4.5 at **$0.80 in / $4 out** — that was Haiku **3.5** pricing. The correct rate is **$1.00 / $5.00**. Minor impact on the blended cost.
- Both errors logged to `~/openclaw/memory/shared/learnings/LEARNINGS.md` as LRN-20260405-001 with the rule: "Never quote $/token from memory; WebFetch the provider page and cite source + date inline."

### Per-workflow unit cost math

For each workflow I compute: `cost = fresh_input_tokens × base_in + cache_write_tokens × 1.25 × base_in + cache_read_tokens × 0.10 × base_in + output_tokens × base_out`.

Parameters per workflow (turns, input/turn, output/turn, cache fraction, model) are picked based on workflow complexity — scheduling with conflict resolution runs 8–10 turns, a simple balance lookup runs 2 turns, a new-patient intake runs 12–14 turns. Cache fractions are 70–80% depending on how much of the prompt is reusable tool schemas and clinic knowledge base vs. fresh patient context.

The single biggest cost line is **voice vendor spend ($33.02/day = 49% of total)**, followed by **Opus 4.6 on inbound call triage ($14.40/day = 21% of total)**. LLM tokens are 32% of total compute cost; voice + infra dominate. **Any optimization should start with voice vendor sourcing** (negotiate Retell volume pricing, consider in-house Deepgram + custom TTS), not LLM tuning.

### Note on the prior "$8.96/day" figure
An earlier version of this report quoted $8.96/day / $188/month. That figure was **LLM-only** and used **Sonnet 4.6 blended** for scheduling workflows. The revised numbers above correct two things:
1. **Model tier**: Scheduling workflows on this tenant use **Opus 4.6 high-reasoning** per Gautam's standing config, not Sonnet. This ~3.6× the unit cost of phone triage and scheduling actions.
2. **Total compute**: Added voice vendor spend ($29.91/day — the actual biggest cost line) and infra fixed costs ($12.50/day) for an all-in number, not LLM-only.

The earlier number understated the total compute cost by ~7×. The $66.99/day figure is the realistic all-in cost, still well within a defensible margin at $2,550/mo.

## 7. Updated cost of goods sold (COGS) and margin

Prior COGS estimate I gave you was $1,700/mo solo and $750/mo at scale — that was **4× inflated**, driven by (a) over-quoting Opus pricing, (b) accidentally folding amortized engineering OpEx into COGS (which is wrong in SaaS accounting — engineering is fixed OpEx, not per-customer COGS), and (c) using volume assumptions 2–3× below reality.

Rebuilt COGS at **verified volumes** and **verified Bedrock pricing** (revised with Opus 4.6 on scheduling and full voice-vendor cost):

**Honest assessment**: margin is healthy but meaningfully tighter than the earlier $188/mo LLM-only estimate would suggest. The dominant COGS line is **voice vendor spend ($33.02/day = 49% of total compute cost), not LLM tokens** — that's the leverage point if margins need to improve. Moving from Retell ($0.12/min) to Deepgram + self-hosted TTS could cut voice cost ~60% ($33.02/day → ~$13/day) and push solo margin from 43% to 60%.

**Break-even at $20k/month engineering OpEx (1 senior engineer + shared infra + G&A)**: contribution margin per solo clinic is $1,091, so break-even = **$20,000 / $1,091 = ~18 clinics**. At 30 clinics you net ~$13k/mo pre-G&A; at 50 clinics ~$53k/mo.

**The "$188/mo LLM only" framing is misleading**. The full compute cost for running a clinic automation agent at this workload is $1,454/mo, not $188. The $188 figure only captured language-model inference; it excluded the voice vendor (which is 49% of total compute) and fixed infra. Any investor or customer pitch MUST use the full compute number, not the LLM-only number, or the first real deployment will blow up the unit economics.

### Three assumptions that could break this

1. **Automation reach** — the cost model assumes the agent actually handles the workflows. Today's honest reach is 54% of billable work (13+ yellow + 1 green out of 26 workflows). The remaining 46% still requires a human, which means the $2,550 buys ~77% of a single shift's work. If the pitch is "replace 1 FTE," the price is defensible because Exult actually runs 1.43 FTEs of work through one person. If the pitch is "replace *all* front-office work," the price is too high because the tail of red-grade workflows still needs a human.

2. **Voice call volume volatility** — the 33/day missed-call backlog depends on the 36.8% miss rate holding. If the automated voice layer drops miss rate to <5% (industry benchmark for a good IVR + voice agent), the callback workflow shrinks to ~4/day and voice vendor cost drops by ~$2/day. The bigger value is the **lift in answered-call conversion**: 2,125 Q1 missed calls × some-fraction that became lost scheduling opportunities is the real operational dividend, not the compute savings.

3. **Self-pay collection risk** — the 85% self-pay patient mix means revenue realization depends on card-on-file collections. Industry benchmark for self-pay collection is 65–75%, not 100%. That doesn't directly affect our per-clinic COGS, but it affects the customer clinic's ability to pay $2,550/mo reliably. Churn risk is higher than insurance-heavy clinics.

## 8. Gaps still open (what's not in this report) — REVISED 2026-04-05 post-agent-completion

Resolved overnight (previously listed as open):
- ✅ **Q1 cancellation count**: 465 Q1 cancellations (6.94/biz day) via REST `/api/scheduler/appointments` status=10 — the list endpoint returns cancelled status; only 5/11 are filtered.
- ✅ **Q1 no-show count**: 286 Q1 no-shows (4.27/biz day) via REST status=12.
- ✅ **Q1 completed visits**: 1,861 (27.78/biz day) from REST status=3 = "Seen".
- ✅ **Q1 new patient count**: 170 (2.54/biz day) via `ppmdmsg getreminderappts` type-name filter.
- ✅ **Q1 appointment confirmations**: 1,152 (17.19/biz day) via REST confirmdate field.
- ✅ **Microsoft Graph inbox volume**: 5,096 messages across 3 mailboxes (exult-info + kendra.geller + raj.bhargava) Jan-Mar 2026 = ~79.6 messages/biz day combined, 133 insurance-keyword matches.
- ✅ **AMD endpoint catalog**: 215 endpoints documented from the authenticated docs crawl, grouped into 23 categories at `.pi/services/amd/API_DOCUMENTATION.md`.

Still open (remaining gaps, all non-blocking for the cost-model decision):

1. **Q1 reschedule count** — status 5 ("Moved") and status 11 ("Deleted") are silently filtered from `/api/scheduler/appointments`. The XMLRPC action `getupdatedvisits` returns 403 "privilege denied" for Gautam's current ADMIN role. **Unblock**: grant the `view updated visits` API privilege in System Settings → User Admin → Roles → ADMIN → API Privileges (5 minutes of clicking). Then re-run `ppmdmsg getupdatedvisits fromdate=2026-01-01 todate=2026-04-04`. Current estimate: ~9/biz day (industry adjacency to the 6.94 cancels/day).

2. ~~**Full Q1 RingCentral call volume**~~ ✅ **RESOLVED** — all 13 weekly windows completed. 5,772 inbound records Jan 1 – Mar 31, 61 biz days, **89.2/day mean**, peak 147 (Mar 2). Miss rate **36.8%** (Accepted 58.7%, Missed 22.2%, Voicemail 14.7%). By month: Jan 1,947 · Feb 1,828 · Mar 1,997 (essentially flat over Q1).

3. **AMD encounter note volume** — `ppmdmsg getehrupdatednotes` returns 403 "privilege denied". Need "view new and updated patient notes for a template" grant. Inferred at 27.78/day (1 per completed visit, standard psych workflow).

4. **AMD billing inquiries (`gettxhistory`)** — legacy action name is wrong for this tenant; returns "Action not found". Need to trace the current action name from the PM webapp's billing section. REST `/api/billing/*` and `/api/charges` both 404. Inferred at 6/biz day.

5. **Portal message triage endpoint** — `PATIENT_MESSAGING` feature is enabled per `/api/system/startupvalues` but no REST path found. `/api/messages`, `/api/tasks`, `/api/inbox`, `/api/portal/messages`, `/api/communication/messages` all 404. `/api/worklist` returns 403 (add worklist privilege to unblock). Inferred at 9/biz day.

6. **Write-endpoint validation for `/connect/v1/*`** — the hipaa-agent codebase has stubs for `/connect/v1/appointments`, `/connect/v1/charges`, and `/connect/v1/patients/{id}/balance` writes, but none have a confirmed 2xx response on Exult's tenant. These are the endpoints that flip the automation ceiling from 54% → ~65% of billable work. Top priority for the next live session.

### The BIG unblock (do this one thing)

**Grant Gautam's role the following XMLRPC API privileges in System Settings → User Administration → Roles → ADMIN** (5 minutes of clicking):
- `view updated visits`
- `view updated patients`
- `view visit information for a date`
- `view new and updated patient notes for a template`
- `view fieldset information`
- `view appointment history`

These six privilege grants turn every "BLOCKED" row in `DAILY_BASELINE.md` into a LIVE measured number. The value of doing this is equivalent to ~3 more background-agent hours of probing — but it's 5 minutes of UI clicking. Biggest ROI move in the entire stack.

## 9. What I recommend next

1. **Next 48 hours:** get the cancel/no-show/reschedule counts from whichever endpoint works (legacy `@action=getmissedappts` is the cheapest to test — single ppmdmsg POST, already documented). Those are the three workflow rows your cost baseline is most sensitive to, and they also directly bound the value proposition of appointment automation.

2. **Pitch reframe:** stop selling "replace 1 FTE for 50% the cost." Sell "augment 1 overloaded FTE, remove the 40% missed-call backlog, and unlock 2× growth headroom without proportional hiring." The numbers support that framing. They don't support the replacement framing until `/connect/v1/*` writes are validated and automation reach climbs past 60%.

3. **Price test (REVISED v2):** at 43% solo gross margin on the corrected COGS, $2,550 is **fairly priced**, not underpriced. The headroom is in voice-vendor cost reduction (Retell → Deepgram self-hosted saves ~$20/day = $440/month, pushing margin to 60% solo). Tiering recommendation: keep $2,550 as the reference "full-service" tier (for clinics matching Exult's workload: 50+ appts/day, 70%+ telehealth, mixed provider panel). Test a **$1,495 "entry" tier** (voice automation + basic scheduling only, ~30% of workload covered, 55% margin) and a **$3,495 "group" tier** (multi-location support, encounter note automation once EHR API validated, 45% margin). Do NOT raise the base tier above $2,550 without validating write endpoints — the pitch requires proving automation reach >60% to justify it.

4. **Instrument before committing to any price card:** the three assumptions in §7 need real pilot data before you lock in pricing. Specifically, instrument the cache hit rate (Bedrock returns `cacheReadInputTokenCount` on every response — fold it into CloudWatch), the per-workflow automation success rate (did the agent actually finish without escalating?), and the customer's self-pay collection rate (tracks their ability to pay us reliably).

5. **Fix the memory that said you were a provider column.** Already done — `reference_amd_api.md` now correctly says Dr. Deepika Bhargava is BHAR00, the primary provider, and you (Gautam) are COO/front-desk, not a clinician.

Independent `/api/scheduler/appointments/{id}` detail pull on a random 400-record sample to recover the `creationdate` field (the list endpoint strips it). Results:

- **71.8% of Q1 kept appointments were CREATED during Q1 2026** (287/400 sample)
- 28.2% were carried over from pre-2026 bookings
- Implied Q1 new-booking volume: **~2,120 appointments created in Q1** = **31.6 new bookings per business day**

This gives us the first live measurement of the "how many appointments does the front desk BOOK per day" metric, which v2 had estimated at 4/day (workflow #3 "schedule new appointment non-new-pt"). **Actual number is ~10× higher at 31.6/day** — a significant undercount.

**Cost-model implication:** workflow row #3 should bump from 2/day → ~29/day (minus new-patient books which are already separate). Applying Opus 4.6 at $0.1392/call × 29 calls/day = **$4.04/day added** to the LLM line. New LLM subtotal: $21.45 → **$25.49/day**. New all-in total: $66.99 → **$71.03/day** = **$1,541/month**. New margin at $2,550: **39.6% solo** (was 42.8%) / **55.3% at scale** (was 57.3%).

Still defensible, still above break-even, but tighter by ~3 percentage points. The v2 report numbers should be read as **lower-bound booking volume**; the real number is ~10% higher all-in than the v2 report states.
